The case I will introduce this time is a patient who had a dorsal hump causing the nasal bridge to appear uneven, along with a bulbous tip and a wide alar base. As a result, the overall size of the nose appeared large, giving a somewhat blunt and heavy impression.
In such cases, simply refining the protruding dorsal hump may not be sufficient for overall improvement. It is necessary to examine the shape of the nasal tip and alar base together and adjust each proportion accordingly. Planning male alar reduction with these aspects in mind is important.
Especially for male noses, rather than overly softening the dorsal hump, I believe it is important to create a line that maintains a masculine skeletal structure and impression, with a relatively straight extension to the nasal tip. This is also considered when determining the surgical approach during consultations.

First, the protruding dorsal hump was refined to smooth out the uneven nasal bridge line. Taking into account the characteristics of a male nose, rather than making it excessively soft or feminine, the correction was made to create a moderate straight nasal bridge, which is an important aspect when planning male noses at KOWON Plastic Surgery.
For the nasal tip, cartilage repositioning, cartilage suturing, and soft tissue removal were performed together to refine the volume of the previously wide and bulbous tip. Additionally, a support structure was placed between the nasal tip cartilages to help maintain the shape of the tip stably after surgery.
To reduce the width of the widely spread alar base, an incisional male alar reduction was performed. The focus was on adjusting the width of both sides and achieving overall balance so that the proportions between the nasal tip and alar base would connect naturally.
Incisional alar reduction is a method that directly excises skin and soft tissue to reduce the width of the alar base. Compared to the non-incisional method, the range of change is clearer, and for bulbous tips with both thick and wide noses, it can be considered as one of the male alar reduction options.
In this correction, rather than simply making the nose appear smaller, the dorsal hump, nasal tip, and alar base were adjusted so that they connect as a single line rather than appearing separate. At KOWON Plastic Surgery, we also determine the surgical approach by considering the overall harmony of the face and nose in this way.
As a result, the previously blunt and large impression of the nose was refined, and while maintaining a masculine atmosphere, it was improved to give a cleaner and more sophisticated look. In particular, the wide alar base was balanced with the nasal tip through male alar reduction, allowing the overall proportions of the nose to connect naturally.

In this case, the nasal tip was thick and bulbous, and the alar base was widely spread to both sides, making the nose appear large and blunt overall. Especially, the tip appeared compressed and the low glabella resulted in a lack of nasal projection.

First, cartilage suturing of the nasal tip was performed to gather the spread nasal tip cartilage, and soft tissue removal of the nasal tip was combined to refine the thick volume of the nasal tip.
The widely spread alar base was reduced laterally with alar reduction, and since the patient wanted a natural line for the low glabella, silicone was used minimally. In addition, the mild dorsal hump was also refined, which organized the overall nasal line.
After surgery, the previously wide and bulbous nasal tip and alar base were slimmed down, making the nose itself appear smaller. The previously blunt impression from the front became sharper, and the case was improved to a natural nasal line that harmonizes with the overall face without being excessively raised.

In this case, the focus was not just on making the nasal tip smaller, but on balancing the previously spread nasal tip, wide alar base, low glabella, and dorsal hump together.
Even for the same bulbous tip, the required correction method may differ depending on the shape of the nasal tip cartilage, the amount of soft tissue, the width of the alar base, and the height of the nasal bridge.
