TYPE 04 — Long & Arrow Nose
Long & Arrow Nose Rhinoplasty
How is a long or arrow nose corrected?
Surgery that lifts the drooping tip to refine the profile. If the tip drops further when you smile, a muscle may be the cause, so we check that too.
Who this is for
- A tip that points downward, looking sharp or aging
- A tip that drops more when you smile and makes the philtrum look short
- A tip that sags toward the lip in profile
A note from Dr. Kim Hyung-taek
Arrow and long noses are among the areas I am most confident about.
Lifting the tip itself is not a difficult operation for anyone. But lifting the tip alone does not keep the lifted tip in place.
A long, arrow-shaped nose often involves muscles that pull the tip down and spread it.
If a physical force drags and presses the tip down every time you make an expression, the lifted tip will droop again that much sooner.
At consultation I ask you to smile, and we operate only when the muscle movement is clear — it is not for every long nose.
Raising the tip also changes the angle between the columella and the upper lip. So rather than stopping at the tip, we decide the design together while looking at a simulation of the whole profile.

How the surgery works

STEP 01
Redirect the tip cartilage upward and fix it with a cartilage graft

STEP 02
Refine the bridge line if needed

STEP 03
Tip-muscle correction only when the pull on smiling is strong
Recovery
Recovery varies slightly depending on the scope of surgery and combined techniques. Below is the typical course.
- 4 days
Cold compress
Apply cold compresses for 4 days and sleep with your head elevated for 5 days.
- Day 5
Stitch removal
Stitches are removed on day 5.
- During recovery
Tip stiffness
A stiff feeling at the tip resolves naturally as swelling subsides.

Outcomes and side effects vary from person to person. Bleeding, infection, swelling and asymmetry may occur. Please consult our medical team before making a decision.
Frequently asked questions
If muscle pull is the cause, tip-muscle correction improves it. If the pull is weak, raising the tip position alone is enough.
If the bridge is high enough, tip-only work is often sufficient. A low bridge can throw off the balance, so we look at both together.
It depends on your face. We decide within the range that avoids creating an upturned look.
