The patient who visited this time was a case where bulbous tip rhinoplasty was definitely needed due to a round and wide-looking nasal tip. The nasal bridge and tip lacked definition, making the nose appear somewhat sunken in the center of the face.
In fact, for this type, simply raising the nose alone makes it difficult to achieve the desired look. You need to address not only the width and direction of the nasal tip and the smooth connection with the nasal bridge, but also the volume during bulbous tip rhinoplasty to achieve a harmonious and refined result.
These days, many people prefer a 'glamorous nose line' that is both natural and striking. However, if you just raise the nose without enough facial definition, the nose can look like it's floating. That's why I focused on balancing the entire face rather than excessively increasing the height.
[Surgical Plan and Method]
I analyzed the overall facial proportions and appropriately built up the nasal bridge with silicone, making careful adjustments to enhance the central definition. For bulbous tip rhinoplasty to refine the wide alar base, I repositioned and tied the cartilage, and neatly removed unnecessary soft tissue. To maintain the shape over time, I placed a firm autologous cartilage support to enhance stability.
I focused not just on narrowing the width, but on creating a smooth curve connecting the nasal bridge and tip. To reduce the heaviness typical of thick-skinned cases, I performed delicate tissue removal along with bulbous tip rhinoplasty to achieve a refined line.

I designed the nose to highlight a detailed yet not excessive line, which is the current trend. Adding definition to the face can make the overall appearance look more polished. I performed the surgery to restore a distinct impression while fully preserving the patient's natural look.

In this case, the nasal tip was short and upturned, so the nostrils were quite visible from the front. Overall, the nose itself was rather small, and there was not enough septal cartilage available for surgery, so the key was to sufficiently extend the nasal tip.

To compensate for the lack of material, I used ear cartilage or costal cartilage to secure a sufficient amount, then clearly extended the nasal tip and columella forward to balance the entire face. In this process, I exposed the alar base that was previously sunken toward the cheeks, and also smoothly corrected the blunt bulbous tip line.

Although it may appear to be simply a short and round nose on the outside, I designed it so that the nasal bridge and tip connect harmoniously by combining delicate correction and nasal tip extension.
Even though there was still swelling immediately after surgery, the previously round and somewhat unsophisticated image was neatly refined. From any angle, the nasal contour revived with a sophisticated and feminine glamorous feel, and even the previously protruding mouth area was naturally refined, resulting in a more luxurious overall appearance.

As in this case, if the nasal tip is short and wide or the bone structure is flat, rather than simply increasing the height, it is much more important to correct the overall proportions and definition.
