Today, I’d like to share a case of revision rhinoplasty for a broad nose, focusing on narrowing the nasal wings and improving the nasolabial angle.
This patient had undergone a rhinoplasty 7 years ago using silicone, septal cartilage, and ear cartilage. Despite the surgery, he continued to experience nasal congestion and suspected a deviated septum.
In the pre-surgery photos, you can see that the nose appears short, with visible nostrils from the front view.

The surgery was performed under sedation anesthesia with an open rhinoplasty approach. We aimed to remove as much of the nasal tip’s soft tissue as possible.
The existing septal cartilage was reused where feasible, and additional ear cartilage was harvested as needed. We also performed nasal wing reduction to minimize the broad nose appearance.
The silicone implant was upgraded from 3mm to approximately 5mm to achieve the desired result.
45-Degree Angle: The nasal tip has been lengthened appropriately, and the nasolabial angle has improved significantly.
Side View: The nasal tip now appears slimmer and lighter compared to its previous rounded and thick appearance.
The nasolabial angle has been refined, enhancing the overall side profile of the face.
Front View: The broad nose has been significantly improved, and the nasal wing reduction has made the nose proportionate and natural-looking.
The photos show some swelling and prominent scars, but the improved nasal shape is already evident.
This case highlights the effectiveness of combining nasal wing reduction with revision surgery to address broadness and achieve a more refined nasal appearance.
